Howard Brown is partnering with Rush University Medical Center, SeniorLife (formally known as Council for Jewish Elderly), Midwest Palliative & Hospice Care Center, and Heartland Alliance to design and develop a comprehensive health program for lesbian, gay, bisexual, and transgender older adults. With support from The Baxter International Foundation and Chicago Community Trust, the Elder Services Community Initiative will create a new model of care for LGBT older adults. We would love the opportunity to chat with members of your organization regarding the Elder Services Community Initiative. We are looking to receive input from LGBT elders. We need to recruit LGBT Elders (ages 60+) to fill out a needs assessment survey in which they can share their experiences and needs forhealthcare programs and other services.
